I'm new to using metal box covers on 4" and 4-11/16" square boxes. All the boxes I've got only have two bolts/threads to secure a cover, while the covers have four holes. For aesthetics, how do real electricians fill the holes to make the cover look finished? Cut off a bolt to just the head and a short nub and silicone it into the hole?

They don't. Those holes are not considered significant for most purposes, and are not required to be filled by electrical codes. A couple short screws with nuts on the inside would probably work to produce the "finished" look you desire.
